* We are considered out-of-network for Blue Cross Blue Shield plans whose mental health benefits are managed by Cigna or Magellan.
If you have an insurance plan that is not listed above, please call the number listed on your insurance card for information about your out-of-network mental health benefits.
Fees and co-pays are collected at the time of service.
The files listed below contain the intake forms and questionnaires that you will be asked to complete when you come for your first appointment. You may print out the forms (Microsoft Word format) in advance, complete them, and bring them with you to your first appointment:
